Vendor Registration

Join us as a vendor at the Dementia Spotlight Day of Education! Secure your spot today for just $50 per table by completing the checkout process after submitting this form. As a valued vendor, we kindly ask that you provide a door prize for the event. Please note that doors open at 8am and vendor tables must be set up by 9am.

This registration form and table fee must be submitted by June 13th. Please note that your vendor registration will not be complete until you finish the Vendor Table checkout process or submit a check via mail. If you encounter any issues with checkout, please contact gary@dementiaspotlightfoundation.org, and a direct checkout link will be emailed to you. If you are paying by check, please make it payable to “Dementia Spotlight Foundation” and the memo line should read “2025 DSF Day of Education.”
